Records Imaging and Subpoenas

Send a written request with a HIPAA-compliant authorization signed by the patient through the verified fax route.

Requesting records

Call (833) 767-5395 to confirm routing, then fax the written request and signed authorization to (858) 560-1974. Attorney requests will be handled as required by California Evidence Code section 1158, with copy charges limited to those allowed by law. Imaging may be provided on disc or by secure link.

Subpoenas

Deposition subpoenas for records are handled by the custodian of records. Call for the current service instructions. Records are produced with a custodian’s declaration.

Patient requests

Patients may request their own records directly at any time using the approved records-request route or at the front desk.
